Position Overview
We are seeking a highly motivated, compassionate, and forward-thinking Nurse Practitioner to join our growing team. This role involves direct patient care with a focus on hormone replacement therapy (HRT), wellness optimization, and chronic condition support. You’ll work collaboratively with a multidisciplinary team and help patients reclaim their health with a root-cause, whole-body approach.
Your Key Responsibilities

- Conduct follow-up visits in person and via telehealth for both HRT and weight loss patients
- Develop and manage individualized treatment plans focusing on hormone balance, thyroid health, metabolic health, and chronic disease prevention
- Interpret labs and explain findings to patients in a clear, educational, and empowering way
- Prescribe and manage HRT (pellets, compounded creams/gels, injectables, oral formulations)
- Educate patients on supplements, nutrition, and lifestyle strategies to support treatment plans
- Maintain accurate and timely charting in EMR (Healthie)
- Collaborate with front office and support team to ensure smooth patient experience
- Participate in clinical meetings and continuous quality improvement processes
- Educate and perform skin health assessments
Your Qualifications
-Valid SC APN license with prescriptive authority
-Valid SC DHEC and DEA registration
-Master's degree in Nursing from an accredited program
-Minimum of 2 years of experience as an Advanced Practice Nurse
-Experience or strong interest in womens and men’s health, testosterone replacement therapy, wellness-based functional medicine, and weight loss.
-Proficiency in electronic medical records (EMR)
-Excellent interpersonal and communication skills.
-Ability to work independently and as part of a collaborative team.
