We are looking for a Support Supervisor/Case Manager to assist adult individuals with Intellectual & Developmental Disabilities (IDD).
HOURS: Full-time, Monday-Friday 8:30am to 5:00pm with required on-call duties.
May require you to work Direct Care shifts when on-call if needed.
Good communication skills (oral, written, and computer skills) are required.
Good organizational skills are a must.

MINIMUM REQUIREMENTS:
Must have at least 5 years of experience in the field of intellectual and developmental disabilities.
Must pass and maintain criminal background check.
Must pass and maintain a good driving record.
Must be able to lift 50 lbs. safely.
Must have/bring in a current TXDL.
Must have/bring in a Social Security Card.
Must have/bring in High School Diploma
REQUIRED JOB DUTIES:
- Maintain consumer records; annual reports & medical records according to state standards. Complete and maintain PDP's, IPC's, ICAP's, IP's
- Manage group homes and house supervisors to ensure the home is cleaned and maintained.
- Oversee consumer needs, and request personal items.
- Communicate and maintain relationships with parents, guardians, house supervisors, and staff.
- Complete annual staffings.
- Assist with staff training, scheduling and working on Direct Care Shifts if necessary.
- Being available for on-call duties.
- Ensure consumers are being provided quality care in and out of the home.
- Ensure staff accurately completes and turn in paperwork in a timely manner.
- Provide transportation to consumers’ Dr. appointments if necessary.
- Complete shopping as necessary.
