Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team at our men's 16 bed low intensity residential treatment home. The ideal candidate will be responsible for providing support and guidance to residents in need, ensuring they have access to necessary resources and services. The Case Manager will play a crucial role in crisis intervention, behavioral health support, and overall needs of the residents. This position requires strong interpersonal skills, the ability to manage crises effectively, and a commitment to promoting recovery from a drug and alcohol lifestyle for residents.
Responsibilities
- Conduct assessments to determine the needs of clients and develop individualized service plans.
- Provide crisis management and intervention services to clients facing immediate challenges.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Collaborate with community organizations, schools, and healthcare providers to coordinate services for clients.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
- Advocate for clients' rights and access to resources within the community.
- Monitor client progress towards goals and adjust service plans as necessary.
- Participate in training and professional development opportunities related to social work and behavioral health.
- Light housekeeping, stocking supplies, and ability to drive and provide transportation to resources.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field.
- Experience in case management or related field with drug and alcohol treatment programs desired.
- Strong skills in crisis intervention and management techniques.
- Knowledge of behavioral health practices and workforce development strategies.
- Excellent communication and interpersonal skills for effective client interaction.
- Ability to work collaboratively with diverse populations and multidisciplinary teams and able to work independently.
- Proficiency in mentoring others within a supportive environment.
- Strong organizational skills with attention to detail in documentation practices.
