Community Horizons is delighted to support a wonderful family in the Plover area as they seek a caring and compassionate caregiver/buddy for their adult son, who has Autism. The goal is to help him stay engaged within the community and at home, while also providing some well-deserved respite for Mom and Dad.

· Wage: $17.00 -$17.50 HOURS: Weekday Afternoons Up to 2 - 3 days 3 hours per day.

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.
