Join our hospice team serving the San Gabriel Valley as a Per Diem Hospice RN focused on SOC (Start of Care) admissions and follow-up. This role gives you the flexibility to pick the visits that work for you while contributing to culturally responsive, patient-centered end-of-life care alongside a coordinated interdisciplinary team.
Requirements (must-haves):
“Bilingual Hospice RN – SGV SOC Admit/Follow-Up”
- Fluent in Cantonese, Mandarin, or Vietnamese (at least one)—language ability is critical for this role
- Current, unrestricted California RN license
- Minimum 1 year of nursing experience (hospice/home health preferred but not required)
- Strong clinical assessment skills
- Consistently complete documentation accurately and on time (same-day or per agreed standard)
- Team-oriented with clear communication; able to coordinate with both in-office and field staff
- Comfortable working independently in the field while staying connected to the care team

Pay: $100.00 per hour
Work Location: In person
