Clinical Manager - Admissions and After Hours
M-F 8:00am – 4:30pm
Position Summary
The Clinical Manager –Admissions and After-Hours is responsible for the leadership, supervision, and operational oversight of the intake, admissions and hour hours care teams that support the initial entry into hospice care and provide 24/7 clinical support. This role ensures that all processes related to patient intake, admission, and after-hours care are delivered with compassion, efficiency, and in full compliance with regulatory and organizational standards. The Manager collaborates with clinical and administrative leaders to optimize workflow, improve patient experience, and maintain continuity of care throughout the hospice journey. This role will provide support as a resource to the Palliative Care program.
Reports to: CEO
Supervisory Responsibilities: This position is responsible for the direct supervision of the Intake/Admissions Coordinator, Admissions RNs, After-hours RNs, and Hospital Liaison.
Responsibilities & Functions:
  • Works in conjunction with the hospice medical directors and hospice CRNP concerning hospice and palliative appropriateness of patients.
  • Promote a culture of accountability, empathy, responsiveness, and continuous improvement.
  • Provides statistics, reports, and records for purposes of evaluation and reporting.
  • Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgment, technical skills, and appropriate knowledge base to lead and guide staff.
  • Provides daily oversight to ensure that hospice and palliative admissions are processed in an efficient and effective manner, ensuring timely patient response and services.
  • Ensures that all referral and admissions activities and processes are functioning in a manner that will optimize growth while in compliance with all clinical operation and regulatory requirements.
  • Offers oversight to ensure patient documentation reflects appropriate diagnosis codes and up to date information in accordance with legal and regulatory standards.
  • Serve as a liaison between intake/admission/after-hours teams and other departments (clinical teams, business development, medical staff).
  • Participate in interdisciplinary team meetings and case conferences as needed.
  • Foster relationships with external partners, including hospitals, skilled nursing facilities, and physicians.
  • Promotes hospice care in the healthcare community. Maintains good standing in the medical community.
  • Provides education to patients and families specific to the hospice benefit, palliative care, end of life care and symptom management.
  • Accountable for meeting patient care needs 24/7 via appropriate scheduling.
  • Completes and delivers performance evaluations in an objective and timely manner.
  • Interviews, hires, coordinates training of new staff.
  • Maintains the confidentiality of employees, patient/client, and agency information at all times.
  • Demonstrate familiarity with policy and procedures of the agency and rules and regulations of State and Federal bodies which aid in determining policy & procedures.
  • Take responsibility and accountability for own nursing practice, continuing education, and professional development.
  • Acts as a resource for billing/finance staff in regard to clinical operations.
Qualifications:
  • BSN or master’s in nursing, Healthcare Administration, or related field, preferred
  • RN degree in nursing from an accredited college or university, required
  • Possession of a current license to practice as a registered professional nurse in the State of Maryland
  • Minimum of three years of hospice nursing experience with management experience, preferred
  • Minimum 2 years of leadership or supervisory experience, preferred
  • Demonstrated ability to work independently with minimal supervision.
  • Knowledge of Medicare, Medicaid, private insurance and other community programs and resources
  • Demonstrated ability to work effectively on an inter-disciplinary team
  • Excellent customer service philosophy and skills required
  • Demonstrated organizational and time management skills
  • Ability to lead teams in a fast-paced, emotionally sensitive environment
  • Excellent communication, organizational, and critical thinking skills
  • Proficiency with EMR systems and Microsoft Office Suite
  • Familiarity with HIPAA regulations and CMS audit processes
  • Experience managing remote or on-call teams

FLSA Status: This is a full-time position and is classified as exempt under the Fair Labor Standards Act.
Schedule:
Monday – Friday 8:00am – 4:30pm (1/2 unpaid lunch break)
Primarily office-based with periodic travel to patient homes or facilities
Must be available for rotating evening and weekend administrative on-call support
Must be available to cover team when and if the need arises
