Description
The Home Visitor (HV) will be responsible for completing all standardized GTP intake forms and assessments. Make referrals and provide follow-up to facilitate continuity of care and collaborate with clients to set goals. Establishrelationshipswithcommunitypartners. Participateincommunitymeetingsandprovide home visits to participants. Oversee the implementation and daily management of the Georgia THRIVe Prevention (GTP) Initiative. GTP is a preventative program designed to support pregnant and parenting individuals, including fathers and caregivers, who exhibit signs or symptoms of mental, emotional, behavioral, or substance use disorders.
Job Responsibilities
- Screen and assess potential clients for eligibility
- Complete intake packet for participant and/or child
- Create and maintain folders of all program participants
- Provide social and psychological support to participants and their families through listening to and discussing problems along with progress as necessary
- Maintain community resources and contacts to facilitate the delivery of services
- Provide follow-up and referral services to facilitate continuity of care
- Assist participants in establishing desirable goals
- Document home visits and other contact with participants
- Assist participants in scheduling clinic or physician appointments
- Participate in community meetings
- Provide presentations to the community on project purpose and/or progress
- Attend community events that serve the demographic population
Minimum Qualifications
- High School Diploma or GED and Two (2) years of experience in a social services-related position OR One (1) year of experience at the lower-level Social Services Tech 1 (SST010) or position equivalent.
- Strong communication skills
