Summary
We are seeking a Habilitation Specialist to join our team at The Hand of Grace LLC. In this role, you will play a vital part in supporting individuals with disabilities by promoting their independence and enhancing their quality of life. This position is essential in delivering our commitment to person-centered care.
Responsibilities
- Develop and implement individualized habilitation plans tailored to each client's needs.
- Provide direct support and assistance in daily living activities to promote independence.
- Monitor client progress and adjust plans as necessary based on data collection.
- Facilitate communication between clients, families, and service providers to ensure comprehensive care.
- Assist clients in developing life skills through educational and job skills training programs.
- Advocate for clients' rights and access to community resources.
Requirements
- Experience working with individuals with intellectual or developmental disabilities.
- Strong behavior management skills and knowledge of special education practices.
- Excellent communication skills for effective interaction with clients and families.
- Ability to collect data accurately for tracking client progress.
- Compassionate caregiving approach with a focus on promoting independence.

Pay: Up to $20.00 per hour
Expected hours: 10 – 30 per week
