About the Position
The Intake Specialist serves as the first point of contact for prospective clients, providing knowledgeable guidance and information about our in-home care services. This role is responsible for managing all communication channels including phone, email, social media, and messaging platforms. The Intake Specialist is dedicated to delivering exceptional customer service and ensuring a positive experience for referral partners, prospective clients, current clients, and their families.

Essential Duties and Responsibilities
- Provide support to the intake team by answering incoming calls on the intake line; engage thoughtfully with prospective clients by asking appropriate questions and delivering accurate, compassionate information about Fedelta Home Care’s services. Redirect inquiries to the appropriate departments as needed.
- Gather and accurately document prospective client information in Axis Care or other CRM systems.
- Conduct daily reviews of prospective clients within the CRM; proactively reach out to existing leads to assess interest, update their status, and ensure timely follow-up.
- Generate and maintain accurate intake pipeline reports; share data-driven insights and emerging trends with the leadership team to support continuous improvement.
- Build rapport with new and existing clients through professional, friendly, and informative communication across multiple platforms, including phone, email, text, and in-person interactions.
- Collaborate cross-functionally with internal teams, including care coordination, home care supervision and marketing, to support the full client lifecycle from initial inquiry to care plan implementation.
- Coordinate and schedule in-home assessments for prospective clients in partnership with Branch Managers or Supervisors.
- Ensure timely and accurate documentation of in-home assessment outcomes within Axis Care or other systems, maintaining up-to-date client records.
- Support client re-engagement efforts; Proactively reach out to lost prospects, or previously inactive, paused, or discharged clients to assess current needs, provide updates on available services, and explore opportunities to resume/start care when appropriate.
- Support off-site activities such as job fairs, community outreach events, and marketing initiatives as needed or requested by leadership to promote Fedelta Home Care and build brand awareness.
- Maintain strict confidentiality of all clients, employees, and company information in accordance with HIPAA regulations and internal privacy policies.
- Establish and maintain trust with clients, families, referral sources, and internal teams through consistent, respectful, and service-oriented communication.
- Assist with follow-ups on assessment outcomes, service authorizations, and intake documentation, as needed, to facilitate smooth transitions into care.
Additional duties may be assigned as needed.

Compensation:
$24-$28 per hour, depending on experience
