Position Summary
The Hospice RN Case Manager delivers compassionate, patient‑centered care to terminally ill individuals and their families. Operating within the continuum of the nursing process—assessment, planning, implementation, and evaluation—the RN Case Manager develops, coordinates, and revises individualized hospice plans of care in collaboration with an interdisciplinary team.
Key Responsibilities
- Conduct comprehensive initial and ongoing assessments, addressing physical, emotional, social, and spiritual needs of patients and caregivers.
- Develop and regularly update tailored plans of care in collaboration with patients, families, physicians, and the hospice interdisciplinary team.
- Coordinate care delivery across settings—patient homes, skilled nursing facilities, etc.—ensuring comfort, continuity, and regulatory compliance Hospice .
- Provide skilled nursing interventions such as symptom management, wound care, IV therapies, and education on medications and equipment .
- Serve as the primary liaison for patients and families, offering emotional support, guidance, and bereavement resources.
- Participate in interdisciplinary team meetings, adjusting care plans based on team insights and evolving patient needs.
- Maintain precise documentation within EMR systems and ensure compliance with hospice regulations and reporting standards.
- Occasionally supervise and support LPNs, aides, or other care staff as assigned.
- Support admissions, on‑call duties, and urgent care coordination as required to meet patient needs.
- Uphold confidentiality, respectful patient care, and professional nursing standards in all situations.
Job Type: Full-time
Pay: $42.00 - $50.00 per hour
