Responsibilities:
- Provide direct care to clients in their homes or in an assisted living facility
- Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
- Help clients with mobility and transferring, including the use of lifts or other assistive devices
- Prepare meals and assist with feeding if necessary
- Perform light housekeeping tasks such as cleaning, laundry, and tidying up
- Provide companionship and engage in social activities with clients
- Monitor and report any changes in client's condition to the appropriate healthcare professionals
- Maintain confidentiality and adhere to HIPAA regulations
- Respect and uphold the rights of residents or clients
Skills:
- Experience in home care or assisted living preferred
- Knowledge of ADLs and ability to assist clients with their personal care needs
- Familiarity with lift equipment and techniques for safe patient handling
- Understanding of resident rights and ability to advocate for clients' needs
- Ability to prepare nutritious meals based on dietary requirements
- Strong communication skills and the ability to interact effectively with clients, families, and healthcare professionals
- Knowledge of social work principles and the ability to provide emotional support to clients
- Ability to navigate care systems and coordinate services for clients
