Overview
We are seeking a compassionate and skilled Home Care Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will possess a strong background in various nursing specialties, including dementia care and critical care transport, and will be dedicated to improving the quality of life for our patients. This role requires a commitment to patient-centered care, effective communication skills, and the ability to work independently.
Responsibilities
- Deliver personalized nursing care to patients in their homes, ensuring comfort and safety.
- Assess patient health needs and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Provide education and support to patients and their families regarding health management.
- Monitor patients’ conditions, documenting changes and reporting to healthcare providers as necessary.
- Assist with daily living activities, including personal hygiene, meal preparation, and mobility assistance.
- Collaborate with interdisciplinary teams in teaching hospitals or memory care facilities to enhance patient outcomes.
- Utilize specialized skills such as administering dermal fillers or managing neonatal ventilators when required.
Experience
- Registered Nurse (RN) license required; experience in home care or community health preferred.
- Familiarity with dementia care practices and memory care environments is highly desirable.
- Experience in critical care transport or working within hospital settings is a plus.
- Knowledge of infant care techniques is beneficial for those working with pediatric patients.
- Strong interpersonal skills with the ability to teach patients and families about health management effectively.
- Proficiency in using medical equipment and technology relevant to home healthcare delivery.
