We are seeking an outgoing, detail-oriented nurse to become our Client Nurse Liaison — the vital link between our clients, caregivers, and care coordination team.
This is a non-clinical, full-time salaried position where your nursing background supports the client journey through oversight, advocacy, and compassionate communication — not hands-on bedside care.
What You’ll Do:
- Serve as the face of Hummingbird Care Services for new and existing clients in our community
- Conduct in-home supervisory visits and client check-ins to ensure care plans are being followed and clients are satisfied
- Build strong relationships with families, offering guidance, education, and emotional support
- Collaborate closely with the care coordination and scheduling team to address client needs
- Update documentation and care plans using digital tools and agency systems
- Identify red flags early and work proactively to ensure continuity and quality of care
- Participate in a rotating on-call schedule to ensure clients always have a trusted resource
Who You Are:
- A Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Florida
- Passionate about senior care and motivated by making a meaningful impact
- Outgoing and personable — you’re the kind of person who lights up a room
- Detail-oriented, organized, and dependable
- A strong communicator with excellent follow-through
- Comfortable with a mobile role that includes travel throughout Indian River, St. Lucie, and surrounding counties
- Proficient with mobile apps, digital charting, and email communication
- Must have reliable transportation and a valid driver’s license
