- Must be a licensed practical nurse (LPN) or licensed vocational nurse (LVN) in good standing and currently licensed by the State. Completion of secondary education or otherwise show ability to read and write in a legible and understandable manner.
- Must be at least 18 years of age. Must be able to read, write, and follow oral and written directions at a level necessary to accomplish the job. Must be able to relate positively and favorably to residents, families, government regulators, and to work cooperatively with other employee at all levels. Must be able to speak and understand English. Must be capable of maintaining regular attendance. Must meet all local regulations and pass post-employment physical exam if required. Must be capable of performing the essential job functions of this job, with or without reasonable accommodations. Related experience at a level necessary to accomplish the job or experience as a charge nurse or supervisory experience preferred.
- Prepare and review medical records and nurses notes, and assess residents to evaluate their physical, emotional status, risk for skin breakdown, and wound management. Monitor resident’s activity and provide nursing care according to the physicians’ orders, care plans, established standards and facility policies. Assist or prepare for new admissions, transfers and discharge for residents. Transcribe physicians’ telephone order and treatments. Consult with physician, DON and Administrator. Provided weekly skin reports to the appropriate staff. Responsible for new admission skin assessments. Provide preventive care to all “AT RISK” residents for skin breakdown. Oversee the need for specialty mattresses, heel protectors, review diet intake, bed rail protectors, oxygen regulators, and other medical equipment to benefit the resident and minimize the risk of skin breakdown. Monitor the turn and reposition schedule. Communicate with residents, family members, and management regarding resident status. Prepare and administer medications and/or treatments as ordered by the physician; in accordance with nursing standards and facility policies. Sign, date and perform all charting and record keeping in accordance with established policies and procedure. Assist and participate in developing and implementing a written care plan for individual residents that addresses the needs of the residents. Any other duties assigned.
- Participate in in-service training, assist in implementing orientation programs and job skills training; maintain professional competence. Provide in-service education to staff regarding skin care, at risk residents, preventive measures, wound management, and turning/repositioning.
- Understand, comply with and promote all rules regarding residents’ rights.
- Access, use or release of resident or facility information is strictly prohibited unless done so in accordance with our facility’s established privacy policies and procedures. It is the responsibility of all employees to protect resident and facility information. Any known or suspected unauthorized release of resident or facility information must be promptly reported.
- All individuals, corporations, agents, consultants, vendors, etc., doing business with or acting on behalf of our facility must understand and adhere to our Corporate Compliance Program Plan.
- Promptly report accidents, incidents, unsafe and hazardous condition and equipment, and ensure that established safety rules and regulations are followed at all times. Monitor environment for established safety, fire and disaster, infection control and all other departmental policies and procedures are followed; and to ensure cleanliness and safety of work and treatment areas.
