Salary: Starting at $24-$30 hourly based on experience.
Statement of Purpose:
This position has two primary responsibilities:
- The day-to-day coordination of the work of the Case Management Team.
- Determining eligibility for and enrolling clients in patient care and other assistance programs, including required documentation and follow up thereof.

Essential Functions:
- Coordinate the day-to-day activities of the Case Management Team.
- Assure appropriate distribution of workload among Case Managers.
- Respond to daily, routine questions and issues of Case Managers and other CAN departments as they relate to patient eligibility, appointments and scheduling/rescheduling, information in EHS and Careware and other aspects of Case Management’s work.
- Assure that new Case Managers are appropriately trained and oriented to patient care programs and participate in screening and interviewing of candidates for new Case Management positions as they arise.
- Provide input to Case Management Supervisor concerning Staff Performance Reviews.
- Assure coordination of client services among Case Managers and with other CAN departments.
- Report any Case Manager performance issues or patient complaints to Case Management Supervisor for appropriate action.
- Screen clients for Ryan White program eligibility, complete required documentation and identify patient needs.
- With client input, develop and monitor progress on individual plans of care.
- Provide Ryan White supportive and comprehensive case management (including financial planning and budgeting, development of life skills, etc., as needed by patients).
- Provide HOPWA services to eligible patients, including: coordination with Lead Agency, Health Planning Council of Southwest Florida (HPC); cross train at least one Case Manager to enroll patients in HOPWA; assure accuracy and completeness of HOPWA paperwork; provide required documentation to HPC when requesting services; maintain spreadsheet of HOPWA expenditures and beneficiaries up to date; make sure Case Managers complete HOPWA referrals spreadsheet and case notes in Careware and provide HOPWA referrals list on a monthly basis to Accounting Dept.
- Provide referrals and coordinate services with peer navigators and other agencies, including federal, state and local assistance programs according to plan of care/patient needs.
- Oversee and coordinate with peer navigators the women’s support group, including planning speakers or other activities, inviting women, ordering food, etc.
- Utilize FL Bureau of HIV/AIDS database program to accurately document all client encounters and services for clients and complete CAN billing sheets.
- With medical team, coordinate with other Case Managers enrollment in Patient Assistance Programs and assure follow up action is provided.
- Comply with all Bureau of HIV/AIDS eligibility and case management requirements, as well as keep up to date with program changes.
- Ensure integrity of case management actions and records through a team self- audit review process.
- Promotes and practices CAN Community Health Inc’s mission and values and follows its policies and procedures.
- Ensures confidentiality is maintained by Case Management Team regarding patient/client information in accordance with HIPAA, professional and departmental standards.
- Cross train for effective client service and team participation.
- Practice safety, environmental and/or infection control methods.
- Participate in professional development activities to enhance knowledge and skills of HIV/AIDS and patient care programs to continually improve client services.
- Utilize resources and supplies in a cost-effective manner.
- Maintain professional, respectful and courteous relationships with clients, staff and visitors.

Requirements
Education/Professional:
- Bachelor’s degree in social services from an accredited school or equivalent case management experience.
- HIV prevention/intervention experience.
- Ryan White eligibility, operations and case management training.
- HOPWA eligibility and procedures.
- Basic Annual Safety Training

- Valid Driver's License

- Familiarity with FL confidentiality requirements for handling client information.
Work Environment:
This job operates in a professional office environment. This role routinely involves using standard office equipment such as computers, audio visual, telephones, photocopiers, filing cabinets and fax machines.
Physical Requirements:
- Neat, professional appearance.
- Sedentary- Exerts up to 10 lbs. of force occasionally and/or a negligible amount of force frequently or constantly in lift, carry, push, pull, or otherwise move objects. Involving sitting most of the time but may involve walking or standing for brief periods of time.
- Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.
Position Type/Expected Hours of Work:
This position requires hours of work outside of normal operating hours.
Travel:
Travel is primarily during the business day, although some out-of-area and overnight travel may be expected. Must be able to operate a motor vehicle and have valid insurance and driver’s license.
