Job Summary:
Under general supervision & in accordance w/ established policies & procedures, reviews requests or subpoenas received from outside providers, agencies, schools & attorneys. Obtains & abstracts charts in response to requests, completes forms & mails information. Assist requestors or patients by telephone or in person.

- Abstract clinical & /or demographic data from patient charts
- Compile medical data from patient charts
- Respond to request for medical information form outside sources
- Obtain authorization to release patient information
- Request charts
- Create graphs & charts
- Collect & compile cumulative data & statistics
- Complete work sheets by tallying activities
- Prepare periodic reports
- Enter data into automated systems
- Perform on line inquiry functions
- Assist patients w/ personal correspondence needs
- Maintain & update user manuals
- Maintain records of committee meetings
- Monitor & maintain protocol books
- Organize & maintain department files, records, handbooks & manuals
- Record & maintain activity log
- Train & orient new or less experience personnel
- Serve as resource person to co-workers by answering procedure question & assisting in problem solving
- Compose memos
- Participate in special projects & studies
- May take dictation & transcribe dictated materials
- May take & transcribe minutes of meetings
- Format & type narrative correspondence & reports
- May type statistical data & reports
- Type check request, materials requisitions & other forms
- Performs other clerical duties as assigned
- Assume other activities & responsibilities as directed
- Works designed HealthConnect work queues as assigned
Basic Qualifications:
Experience
Minimum of three (3) years of relevant clerical experience- Knowledge of medical terminology
- Knowledge of PC & Office applications
- Typing 45 WPM
- Ability to use a computer keyboard to perform assigned duties
- Able to read, write & follow oral & written instructions
