Position Summary
The Optometrist provides professional comprehensive eye care services to pediatric and adult patients of the Lawndale Christian Health Center. They provide quality patient-centered care. The Optometrist will report directly to the CCO of Operations.
Principal Duties and Responsibilities
- Provide quality eye exams and corrective lenses as needed for LCHC patients
- Examine patients in an efficient and timely manner
- Comfortable with medically based optometry, management of diabetics, glaucoma, urgent anterior segment and retinal disease management and referrals.
- Communicate with primary care providers as needed.
- Provide clinical instruction to optometry students.
- Participate in peer/chart review, performance evaluations, and quality improvement efforts.
- Attend programmatic meetings
- Participate in continuing education, including Life Support Training, to maintain the skills necessary to perform Optometrist duties as required by the State.
- Maintain the physical health required to perform duties with the ability to stand and bend 85% of the time; maintains current immunizations and up-to-date PPD.
- Maintains communication with Administration regarding personal issues related to drugs, felonies, discipline, loss of license or privileges, litigation where the provider or clinic is harmed or at risk, or other serious issues.

Compliance and Documentation
- Confirms patient identity according to current procedure before rendering any service
- Thoroughly, accurately, and promptly documents all service rendered in the electronic medical record for every patient.
- Performs all tests, procedures, and other services according to the policy and procedure of LCHC, notify the appropriate supervisor in a timely manner of instrument or procedural problems.
- Knowledgeable of and in compliance with CLIA, OSHA, and JCAHO regulations for safety, infection control, equipment operation, and other applicable areas.
- Participates in proficiency testing as required by LCHC or departmental policy to ensure that proper standards of care are being maintained
- Maintains confidentiality of patient health information in accordance with HIPAA and other applicable regulations.
Required Knowledge, Education, Experience or Licensure
- The Optometrist must be currently licensed to practice Optometry in the state of Illinois. Must document completion of O.D. degree.
- Five-years’ experience or residency training preferred.
