Job Description
Key Responsibilities:
- Provide optometry services for residents consisting of consultation, examination, and prescription and provision of glasses and corrective lenses.
- Make a minimum of two (2) visits to site during the subcontract year as may be required to provide professionally acceptable standards of quality care and treatment to all cases undertaken. The number of hours to be provided during each visit will be 80 hours. The number of visits is subject to change due to unforeseen circumstances. The minimum duration for each SUBCONTRACTOR visit will be of such length so as to accommodate all programmed case appointments. The trips will be planned at least 30 days in advance.
- Supply safety glasses, at the patient’s expense, meeting the requirements of ANSI Z87.1-2003 (Z87+). Lenses for safety glasses shall be made of polycarbonate materiel and contain a scratch resistant coating. Frames shall come equipped with protective side shields.
- Completes required organizational compliance education, including assigned requirements that are client-specified, for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.

Qualifications
Basic Requirements/Certifications:
- Shall have and maintain a current license to practice Optometry in any one of the 50 states, the District of Columbia, Puerto Rico, Guam or the U.S. Virgin Islands that allows for the independent practice of clinical services.
- Minimum of three (3) years of experience as Optometrist.
- Current American Heart Association Basic Life Support (BLS) certification.
Education Required:
Doctor of Optometry (DO) degree from a 4-year program at an accredited optometry school.
Physical Requirements:
- Work is normally performed in a typical interior/office work environment.
- Work involves sitting and standing for prolonged periods of time.
- May require bending, stooping and lifting up to 15 lbs.
Other Special Qualifications:
- Must be able to read, write and speak English to effectively communicate.
- This position is TDY (temporary duty)
- US Citizen or Permanent Resident Card is required.
- US DOD CNACI or the ability to obtain one is required.
- A valid passport is required and must be in your possession.

Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
