Job Summary:
The Palliative Clinical Care Coordinator will be responsible for coordinating and managing the comprehensive care of patients with serious illness or at the end-of-life, ensuring that they receive the appropriate services and support in alignment with their personal goals and preferences. The role involves close collaboration with interdisciplinary teams, patients, families, and healthcare providers to ensure a high standard of care and comfort, while promoting quality of life for patients in palliative care.
Key Responsibilities:

Patient Care Coordination:
- Serve as the primary point of contact for patients and families, guiding them through the palliative care process.
- Assess and manage patient needs, including symptom management, psychosocial support, and spiritual care.
- Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals to develop individualized care plans.
- Monitor patient progress and ensure that care goals and treatment plans are met according to patient preferences and medical indications.
- Provide education to patients and families on palliative care services, advanced care planning, and symptom control.
Qualifications:

- Active LPN license
- Bachelor’s degree in Nursing, Social Work, or a related field.
- Certification in palliative care or hospice (e.g., CHPCA, APN-BC) preferred.
- Master’s degree in a related field is a plus.
