Essential Duties and Responsibilities (This is not an exhaustive list of all duties/responsibilities. Other duties may be assigned)

  • Provide a variety of case management, skills training, support services as required for persons with developmental disabilities, severe and persistent mental illness, and chemical dependency
  • Connect clients with community resources (i.e. housing, food stamps, insurance, social security, etc.)
  • Advocate for benefits on behalf of client
  • Provide a variety of life skills training to clientele
    • Ability to operate office equipment (computer, copy machine, scanner, etc.)
    • Ability to multi-task, manage time, meet deadlines, and remain calm and professional under pressure
    • Maintain standards of confidentiality of Lifeways’ clients and records
Qualifications

Required:

  • BA degree in social work, behavioral health, or related field
  • Two years of experience with individuals with severe or persistent mental illness, or alcohol and drug dependency
  • Or a combination of education and related work experience
  • Valid driver’s license
  • Appropriate state licensure
  • Successful completion of background check

Preferred:

  • Bilingual/multilingual
  • Experience with electronic medical records and telemedicine
