Summary:
The QP Supervisor (Designated Manager) is responsible for oversight of All Home Health's compliance with 245D statutes in Minnesota. The QP Supervisor (Designated Manager) will train and supervise the Qualified Professionals (Designated Coordinators) to ensure they are following 245D statutes and ensure compliance therein. Other tasks include but are not limited to; overseeing client compliance related to openings, annual paperwork, trainings and 245D visit completion, QP time sheets and documentation submission for each visit, managing after visit documentation and filling of completed documentation, communicating with case managers about visit follow up needs or requests for increases, or county with regards to needs, adding new services for clients and ensuring proper documentation and training is provided, problem solving as well as interpersonal skills. QP Supervisor (Designated Manager) is an in-office role and may only be done remotely under extenuating circumstances. Performing these duties will allow the company to maintain client compliance and continue supportive relationships between clients, caregivers and the office.
Key Responsibilities:
- Biannual review of 245D statutes to ensure compliance with changing laws.

Train and supervise the Qualified Professionals (Designated Coordinators) who are responsible to:
- Ensure AHH's duties relating to client's care and outcomes are carried out
- Train and ensure the competency of direct support staff (caregivers).
- Evaluate direct support staff (caregivers) effectiveness in client care and outcomes.
- Ensure All Home Health forms, policies, and procedures are in line with 245D standards.
- Review/complete corrective actions items, incident reports, and emergency reports to ensure continual improvement of policy and procedures.
- Visit clients in their homes as required for supervision and backup of the Qualified Professionals (Designated Coordinators).
Qualifications:
- Education (must meet one of these):
- a bachelor's degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to people with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or
- an associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to people with disabilities or people age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or
- a diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to people with disabilities or people age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or
- a minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or people age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1a) to (1c).
- Work Experience (must meet both):
- Have a minimum of three years of supervisory level experience at a company that provides care or education to vulnerable adults or children and
- a minimum of 2 years' experience working specifically through 245D Basic or Intensive support services either managing staff who complete or completing themselves support plans and other required 245D documentation.
