RN Case Manager – Private Duty / Medicaid Waiver Programs

American Hearts & Home is seeking a skilled and compassionate RN Case Manager to oversee care for clients receiving Private Duty Home Care and services through Medicaid Waiver programs (EDWP, CCSP, SOURCE). This role includes client assessments, plan of care development, and supervision of caregivers to ensure compliance and quality care delivery.

Key Responsibilities:
- Conduct RN assessments and supervisory visits
- Develop and update service plans
- Ensure Medicaid waiver compliance
- Supervise and train caregivers (CNAs/PCAs)
- Maintain accurate documentation in compliance with state standards
Qualifications:
- Active RN license in GA
- 1+ year of nursing experience (home care or waiver programs preferred)
- Reliable transportation + valid driver’s license
- PPD
- BLS
Structured per-visit pay, e.g.:
- $100–110 for start-of-care visits
- $45–55 for routine or supervisory visits
- $0.50–$0.67/mile reimbursement
