Social Worker/Administrator (QIDP)
Under the direction and immediate supervision of the Program Director the QIDP is responsible for the operation and supervision of 3 ICF/DD-H and/or DD-N facilities. Coordinates the development, implementation, and documentation of the individuals’ Program Plan. Manage all staff assigned to the home. Participates in the staff training programs and represents the Agency in leadership and administrative activities. Ensures a positive, normalized home environment.
Description of Duties and Responsibilities:
1) Provides for the full and consistent implementation of each individuals program plan, and instructs Direct Care Staff (DCS) in the proper methods of conducting individualized programs and activities. Monitors daily activity schedule of clients and assures the appropriateness of rising and bedtime hours, bathing practices, meals and meal-time settings, clothing and attire, and matters of individual and home safety, etc. Encourages clients, on a small group basis, to engage in social recreational activities at home as well as in the community.
2) Responsible for ensuring a safe, clean, and smoke-free home environment: monitors the overall physical condition of the home and external environment: encourages client and or his/her guardian to participate in personalizing and decorating his/her bedroom. Provides opportunities for clients to learn housekeeping skills including dusting, dishwashing, bedmaking, etc. Responsible for initiating-repair and maintenance requests, conducting regular home and environmental surveys.
3) Provide direct services for at least 1.5 hours per week per client in an ICF,DD-H and for at least1.75 hours per week per client in an ICF/DD-N.
4) Advises the Program Director of any client-related nursing, medical, or physical health problems, Monitors the implementation of any special dietary requirements for assigned clients.
5) Assigns work schedules and evaluates the performance of DCS: is responsible for accurately recording and monitoring staff attendance; monitors the visitation practices of clients and staff assures the appropriate use of the home telephone.
6) Drive to and work in multiple facilities, schools and day programs.
7) Conducts regular facility meetings, which include staff and clients as appropriate, communicates Agency policy and procedures and provides minutes of such meetings for in service documentation.
8) With clients, plans and monitors community activities and field trips; consults in developing a plan for community experiences.
9) Physically intervene with clients who are exhibiting maladaptive behaviors including hitting, scratching, biting, hair-pulling and pushing Which may include but not limited to hitting, scratching etc.
10) Works collaboratively with the staff of the Day Programs in seeking to maximize client growth and personal effectiveness.
11) Prepares and oversees the preparation of applicable reports and records in a timely fashion. Ensures acceptable completeness, clarity and spelling in the preparation of such documents. Assumes responsibility for tracking and charting such issues as injuries, clothing inventories, specific program needs of the clients or issues within the home such as seizures, appointments etc.
12) Communicates with parents, staff, visitors, community agencies, etc. in a professional and courteous manner. Meets with parents, friends, or guardians of the clients on a regular basis.
13) Functions as a member of the Angel View Leadership Team and is actively involved in residential programs and agency-wide committees and task groups: works with other Angel View staff' members in developing and suggesting event and systems improvements, anticipates potential problem areas, and communicates any identified problem areas or concerns to the Program Director.
14) May participate in the selection/employment of DCS and may provide pre-service and on-the-job training experiences.
15) May participate in the review of the facility budget and related financial tracking. Recommends cost-saving techniques where applicable. Monitors the appropriate use of supplies and materials.
16) May provide, arrange, oversee or ensure the transportation of clients to and from recreational, social, work, medical or other appointments as necessary.
17) Seeks opportunities for self-improvement through reading relevant clinical and management materials. Shares these ideas and suggestions with Angel View colleagues.
18) Works with DCS in encouraging their personal growth and knowledge.
19) Assures the consistent compliance of the individual facilities to State and Federal regulations.
20) Other duties as assigned.
Job Requirements
Meets the definition adopted by the Department of Developmental Services for Qualified Intellectual Disabilities Professional (QIDP). QIDP is responsible for integrating, coordinating, and monitoring each client's active treatment and possesses:
(A) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
(B) Is either a physician and surgeon or an osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology.
