We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will possess a strong background in behavioral health and mental health counseling, with the ability to provide comprehensive support to individuals and families. This role involves conducting patient assessments, developing treatment plans, and delivering both individual and group counseling services. The Social Worker will play a crucial role in enhancing the well-being of clients through effective case management and therapeutic interventions.
Responsibilities
- Conduct thorough patient assessments to determine needs and develop appropriate treatment plans.
- Provide individual and group counseling sessions, utilizing techniques such as behavioral therapy, psychodynamic therapy, and Christian counseling as needed.
- Facilitate intake processes for new clients, ensuring all medical documentation is completed accurately and in compliance with HIPAA regulations.
- Maintain detailed medical records and documentation of patient progress, adhering to medical terminology standards.
- Collaborate with interdisciplinary teams to coordinate care and ensure comprehensive support for clients.
- Engage in case management activities, including follow-up on client progress and adjusting treatment plans as necessary.
- Offer support for aging issues counseling and geriatrics, addressing the unique needs of older adults.
- Participate in group therapy sessions, fostering a supportive environment for shared experiences among clients.
- Stay informed about best practices in social work and behavioral health to enhance service delivery.
Job Details
- Weekdays day shift 8-4:30
- Familiarity with use of electronic medical record such as EPIC
- Individual case management and knowledge of managed care
- De-escalation/restraint management techniques
- Motivational Interviewing and short-term treatment techniques-Brief family interventions as needed
- Strong ability to assess and evaluate patient needs and develop treatment plans to address the identified needs.
- Good communication, listening, and interpersonal skills
- Strong organizational skills
- Ability to be flexible in a fast-paced work environment
- Flu vaccine required
- Current staff will need to be vaccinated
- Active MA license required
