A Case Manager/ Support Coordinator’s duty is to support individuals that have a developmental disability to lead a meaningful life in the community of their choosing. They help coordinating supports in the home, community, health, employment and/or day program. The Support Coordinator will be required to work with representatives within state agencies as well as private service providers within the community to capture the individual's life and supports through documentation. Each Case Manager will be responsible for carrying a caseload of 40-60 participants.
Case managers/Support Coordinator will be engaging in the following activites:
- Monthly phone contacts with the participants
- Schedule visit the participants in their home and ensure health and safety
- Schedule and complete a yearly plan of care with participants, providers and family to ensure that the participants goals are met.
- Problem solving
- The case manager will drive to the participants home. At no time will the Case Manger transport the participant.
There is a milage supplement of $275 a month for driving. We also offer a quarterly bonus for meeting all company deadlines.
Case Managers hours are M-F 8:30 to 4:30, with occasional after hours appointments.
The Support Coordinator is allowed flexibility to work from alternative locations after all in office training is completed.
Requirements:
Applicants are required to have earned a Bachelor’s Degree in a human service related field such as psychology, criminal justice, education, social work, or sociology or General studies with 16 hours of the psyschology, criminal justice, education, social work or sociology.
Applicants must have a vehicle and valid Louisiana car insurance.
