Job Summary: Visiting Nurse (RN/LPN)
We are seeking a compassionate and dedicated Visiting Nurse to provide high-quality nursing care to patients in their homes. The Visiting Nurse is responsible for assessing patient needs, implementing individualized care plans, administering treatments, and coordinating with other healthcare professionals to ensure optimal outcomes. This role requires excellent clinical judgment, strong communication skills, and the ability to work independently in various home care environments.
Home visits will be conducted across all five New York City boroughs—Brooklyn, Bronx, Manhattan, Queens, and Staten Island—as well as Nassau County.
Compensation for this position is made in accordance with the DEEM (Direct Employment Expense Model), which ensures fair and consistent payment for all authorized visits and services rendered.
Pay: $65.00 - $95.00 per hour
