Job Description:
The LPN, under the direction of the Registered Nurse, performs selected nursing care for the sub-acute, convalescent and chronic client.
May assist the client in learning self-care techniques. Will complete appropriate clinical and progress notes.
The Licensed Practical Nurse supports and fosters the philosophy, goals, and objectives of the agency.
Reports to: Supervisor Clinical Services
Minimum Requirements:
- Graduate of an accredited Licensed Practical Nurse program.
- Current license as a Licensed Practical Nurse in Connecticut.
- Minimum of one (1) year clinical experience in a healthcare setting
- Ability to respond to needs of patients and families in varied settings.
- Ability to make meaningful observations and write comprehensive accurate reports.
- Proof of satisfactory completion of a medication course.
Working Environment:
Home visits to patients to include, weekends and holidays and consistent with agency policies and practices.
Sitting, standing, walking, bending, lifting.
Potential exposure to blood and body fluids.
High-stress environment related to dealing with patient illness.
