We are seeking a dedicated and compassionate Case Manager to join our team. Responsibilities
- Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
- Collaborate with healthcare providers, families, and community resources to coordinate services.
- Maintain accurate documentation of patient progress and treatment plans in compliance with HIPAA regulations.
- Collect and analyze data related to patient care and outcomes.
- Provide support and guidance to patients and their families regarding available resources and services.
- Advocate for patients' rights and needs within the healthcare system.
- Monitor patient progress and adjust care plans as necessary to ensure effective treatment.
- Stay informed about developments in medical terminology and best practices related to behavioral health.
Qualifications
- Bachelor's degree or higher in Social Work or related field
- Previous experience working with individuals with developmental disabilities
- Strong program management skills
- Excellent relationship management abilities
Duties
- Provide case management services to individuals with developmental disabilities
