Qualifications:
- Possess a Master’s degree from an accredited college or university with a major in Social Work or Counseling
- Hold licensure as a Licensed Master of Social Work (LMSW) or Licensed Professional Counselor (LPC) from the State of Michigan.
- Have had at least three (3) years of full-time casework, therapy, or assessment experience including 1 year in a leadership role.
Essential Functions:
- Engages individuals and families in a welcoming, hopeful, empathic manner regardless of stage of readiness and phase of treatment.
- Conducts and attends daily team meetings to review problems and plan solutions and completes meeting minutes on a rotation basis. Attends meetings with ACT psychiatrists at least twice per week to ensure continuity of individual care.
- Ensure that team members meet with individuals within 72 hours of a psychiatric admission to facilitate discharge planning.
- Assists individuals with medication compliance and distributes medication vials to individuals as ordered by the psychiatrist, as well as “eyes on meds”, to ensure compliance with treatment plan.
- Ensures and works within the rotation of ACT individuals to ensure that individual needs are met by all professional disciplines.
- Ensures that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals MORC policies and procedures.
- Maintains an up-to-date knowledge and understanding of Medicaid ACT criteria and individual eligibility.
- Assists staff in improving and updating professional skills by providing them with available resource information and communicating current and new program policies and procedures. Orients and trains new staff assigned to the team.
- Ensures that all team members attend annual ACT trainings as well as local in-services.
- Provides supervision in accordance with the individual and clinical guidelines for the position.
- May also be responsible for the coordination of specific programs and projects as assigned.
- Responsible for authorizing medically necessary services for all individuals.
- Screen for co-occurring mental health and substance abuse disorders. Implement integrated strength based assessment to assess the presence, onset, symptoms and patterns of substance use, behavioral and psychiatric disorders. Assess for acute needs and risk and provide/arrange for appropriate stage wise interventions. In the presence of two disorders, evaluation will be made of the interaction between the two, as well as the relationship of each to significant life events. Demonstrate the knowledge of diagnostic criteria for psychiatric and substance use disorders.
- Demonstrate the ability to identify stage change and use interventions consistent with stage of treatment. Assist staff in developing strength-based, stage wise treatment matched person-centered plans that are designated to address identified individual needs. Able to provide resources for integrated treatment including self- help and peer support.
- Model and use the Spirit of Motivational Interviewing in all interactions with staff, individuals and stakeholders.
- Provide outcome and strength-based supervision to staff utilizing a range of supervision techniques to foster professional development and performance, develop skill sets and increase individual outcomes. Conduct monthly case reviews that are competency based, strength based, culturally relevant and focus on improving individual outcomes. (i.e. utilize motivational interviewing with staff-direct, follow, guide approach)
- Familiar with a variety of outcome measures for psychiatric and co-occurring substance use, medical or other disorders. Able to utilize outcome measurements in supervision and person centered planning.
- Provide support, education, and reinforce the use of prescribed medications to treat substance use and mental health disorders.
- Full compliance with developing and monitoring PIP’s in accordance with the Supervision Policy.
- Monthly supervision/coaching requirements met for all staff assigned as defined by Supervision Guidelines.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist consumers with trauma related issues.
- Monitors case records, including signatures, and follow up and referral of individuals to community agencies to aid in an accurate history. Ensures that paperwork is maintained in accordance with Medicaid and DCH, and Easterseals MORC guidelines, policies, and procedures.
