Definition
The BH Care Coordinator (BHCC) is a non-licensed professional with a strong background in mental health. The BHCC is a core member of the BH team with responsibilities in two areas of focus: Patient Care and Integrative Care. Patient Care includes the following: identifying and addressing social determinants of health, providing case management for patient needs through referrals, providing psychoeducation for improved quality of life (individual and group) and advocating patient needs. Integrative Care includes being a core member of a collaborative care team of primary care provider, consulting psychiatrist, dental provider, other behavioral health providers and HW staff. The BHCC will function as a bridge between the patient and care services within HW and community resources, facilitate successful delivery of BH services and provide general support to the behavioral health team.
Roles & Responsibilities
Patient Care -
- Provide case management services for BH patients/team including appropriate referrals and coordination of care with community partners such as CSB, MHDDSA, DSS, LAWS, and others as needed after securing release of information
- Collaborate with the patient and BH Team to support patient with meeting identified case management and care coordination needs via in-person meetings, follow up phone call and email communication
- Assist patients with filling out applications as needed and coaching them on follow-up.
- Develops and maintains relationships with outside referral sources to ensure that BH team has up-to-date information about who is accepting new patients, the process for admission, and who qualifies for which service.
- Work with community partners to identify resources for patients and ensure that the BH team always has adequate up-to-date referral resource information readily available for patient and provider education.
- Provide/conduct behavioral health screenings such as Social Determinants of Health and gaps in services to intervene and link patients to potential community resources for resolving patient health, psychosocial or financial issues.
- Provide patients with self-management tools and psychoeducation to help them cope with behavioral health issues and identify healthier lifestyle choices on an individual basis or in a group
- BHCC will advocate for patient?s best interest and make recommendations for patient special needs such as interpreter services; identified barriers to care such as transportation, medication assistance, outreach, and health education; influence patient?s engagement in their care; promoting a continuum of safe, quality, and cost-effective patient care.
- Monitor specialty and primary care appointments for the patient and assist patient in keeping those appointments (reminders, transportation arrangements, etc.) as needed.
- Support Provide patient with community and agency education related to behavioral health care issues and the relationship to primary care and dental needs.
Integrative Care ?
- Attend and participate in monthly BH team meetings.
- Attend and participate in internal and external meetings either with, or on behalf of the BH Director and the BH team.
- Ensure all referrals sent to BH are processed in a timely fashion and that urgent referrals are processed within 24-48 hours or as soon as possible.
