Purpose:
This position assists clients in maintaining independent living by working with the Occupational Therapist, Project Manager and Program Manager to identify and address home modifications and supplemental programs that result in safe and successful home living.

Compensation: From $23.07 (Dependent upon relevant education, experience, and preferred qualifications). Since this position requires being bilingual , the candidate must achieve a passing score on a standardized /Listening & Reading/Writing fluency exam.
Major Duties & Responsibilities:
- Assists potential clients with registration and income qualification for the OAHMP Aging Well at Home program.
- Attends initial client home assessment.
- Conducts regular check-ins with clients and assists with referrals and registration for Healthy Living workshops, other PCOA services and community services.
- Assists with training clients on the use of newly installed equipment or modifications to their homes.
- Supports the Occupational Therapist’s identification and implementation of strategies to support independent living and communicates with clients about plans.
- Participates in the development of treatment plans and assists in providing clear, ongoing communication with the client.
- Coordinates follow-up with clients to confirm that contractors have scheduled work and that any barriers have been identified and addressed.
- Maintains consistent communication with clients through phone, email, or home visits to support ongoing needs around OAHMP modifications and fall prevention activities.
- Administers required client surveys to gather feedback on services and client satisfaction.
- Attends and actively collaborates with others at all scheduled meetings and trainings.
- Builds and maintains a cooperative working relationship with others.
- Utilizes technical expertise to complete all assigned tasks with an emphasis on quality and timeliness.
- Works independently and collaboratively with others with minimal supervision.
- Performs tasks at a proficient level and handles more difficult, complex, or irregular tasks.
- Ensures that work adheres to local, state, and federal governmental regulations, and other applicable rules and requirements.
- Uses virtual collaboration platforms to perform work as necessary (e.g. Zoom or Teams).
Qualifications:
An equivalent combination of education and experience related to the position may be considered.
- A Bachelor's degree in Social Work, Human Services, or a closely related field; a Master's degree is preferred.
- At least two (2) years of experience in case management and/or social services; three (3) years is preferred.
- Bilingual in English and Spanish is required.
Additional Qualifications:
- Certified Aging in Place Specialist (CAPS) certification required (CAPS training provided).
- Must have reliable transportation.
- Must successfully obtain and maintain AZ Level 1 Fingerprint Clearance and Central Registry background clearance.
Knowledge, Skills, & Abilities:
- Knowledge of case management/care coordination practices, rules, regulations, and requirements.
- Knowledge of the aging process, issues, and health problems associated with aging and disabilities.
- Skill in using Microsoft Office suite and virtual collaboration platforms.
- Ability to demonstrate a commitment to the vision, mission, and values of Pima Council on Aging and its family of non-profit companies.
