If yes, join the great team of people at Koinonia Family Services in Tustin working in our enhanced care management (ECM) program as a part-time ECM Care Coordinator. As a Care Coordinator, you'll play a key role in connecting individuals to services and resources tailored to meet their unique needs. Your responsibilities will include conducting assessments, developing individualized care plans, overseeing service coordination and referrals, and providing education to help individuals make informed, healthy lifestyle choices. We are seeking candidates with strong bilingual or multilingual proficiency to ensure effective communication with our diverse client base.

Opportunity Highlights:

Enhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator will lead the assessment, care plan development and ongoing care management of eligible participants under the supportive guidance of an ECM Supervisor. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members or other adults. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.

The Care Coordinator opportunity includes:

  • Hourly pay rate: Starting wage of $23.00, or higher with prior ECM experience

This is a part-time role offering 20-25 hours per week. Shifts typically run from early afternoon to early evening. There is some flexibility in the schedule to accommodate your personal schedule.

Key Responsibilities:

Under the supportive guidance of the ECM Supervisor, you will work with the ECM team to provide services to youth and families as follows:

  • Conduct assessments of individuals' needs and formulate individualized care plans
  • Enthusiastically educate and enroll clients about services, and motivate them to engage in services.
  • Meet with individuals in their homes to facilitate services
  • Oversee and ensure the seamless coordination of services, including physical, mental, dental care and social services
  • Act as the primary liaison between individuals and their healthcare providers, and social services
  • Stay informed about community resources and actively refer individuals to appropriate services as needed
  • Maintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomes
  • Advocate for the needs and rights of clients within healthcare and social service settings
  • Provide clients and families with resources and guidance on making informed health and lifestyle choices
  • Document services according to ECM program standards
  • Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery.

Qualifications:

  • A strong desire to help people in need achieve successful outcomes
  • Proficient bilingual or multilingual abilities are a plus
  • Minimum 2 years of case management (or related) experience
  • Knowledge of community resources
  • Excellent interpersonal and communication skills
  • Comfortable talking to people and motivating them to seek services
  • Ability to collaborate with interdisciplinary teams
  • Experience with high-needs populations
  • Proficiency in electronic health records a plus
  • A high degree of integrity, ethics and confidentiality
  • Demonstrated ability to work independently and maintain productivity with minimal supervision
  • Cultural sensitivity and awareness
  • Valid driver’s license and auto insurance

We are seeking candidates who have at least two years of related work experience in social work, case management, community engagement or customer service. High school graduate or GED required. Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus.

This position requires a valid driver’s license, auto insurance and reliable transportation as driving is an essential job function. ECM Care Coordinators are responsible for tasks such as visiting client homes, attending in-person meetings, and engaging with community partners, often in locations not easily accessible by alternative transportation methods. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the youth and families we serve.
