Growing Home Care company located in Bergen County, NJ , is looking to hire a full-time Care Coordinator, Scheduler for this fast-paced position. Qualified candidates will be energetic, patient, and attentive to the needs of our clients.
Job Description:
* Answer incoming calls in a friendly, professional and knowledgeable manner.
Create and maintain client and caregiver schedules. Problem-solving skills are needed for creating compatible matches and the development of positive relationships.
- Monitor and log all client and caregiver activity utilizing the software system.
- Follow up with all client and caregiver issues to ensure their problems are resolved in a timely manner.
- Recognize and capture opportunities to increase service hours to enhance and/or increase quality care..
- Maintain open and effective communication with coworkers, caregivers, clients, and family members.
- Misc. case work as needed
- Ensure accuracy of data/information in system and files
- Motivated to resolve and manage any tasks necessary to resolve matters related to patients and/or caregivers
- Keeps schedules up to date with team member changes and communicates any changes in schedules, including weekend coverage, holiday coverage and all vacation coverage;
- Demonstrates excellent customer service through telephone etiquette and timely response to emails/correspondences
- Performs other duties as needed
