The PEM Care Management department is
made up of a diverse group of individuals. Each come with their own unique
backgrounds, skills, and experiences. The department is comprised of Social
workers, RNs, and CMAs. Together we have built a team that can draw on each
others strengths and supplement each others weaknesses. We expand each
others knowledge and skills through collaboration and coaching, and provide
opportunities for growth within the department and organization. We work
together as a team every day to expedite the most appropriate and timely
discharges for our patients. We work collaboratively with other members of the
interdisciplinary team, as well as post-acute providers to ensure continuity of
care. We do this and so much more everyday as a team!
RESPONSIBLE FOR:
Responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.
MINIMUM EDUCATION REQUIRED:
Graduate of Nursing Program
MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in an acute or post-acute setting
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current unrestricted registered nurse (RN) license, or eligible for licensure, in the state of Georgia, required.
ADDITIONAL QUALIFICATIONS:
Certification in Case or Care Management; experience using InterQual or Milliman; Master's degree preferred.
