We provide onsite geriatric primary care services in addition to our physical, occupational, and speech therapy offerings. Position Summary:
As the Care Navigator, you will assist in the coordination of patient care in an assigned community or communities. You are responsible for providing administrative and clinical support to the medical providers and the integrated services team. This position requires independent decision making with limited oversight.
Responsibilities:
Essential Duties include the following:
  • Obtains consent and all necessary intake paperwork from resident or POA prior to initiation of all integrated services
  • Performs data entry tasks and maintains patient electronic health record
  • Documents residents' medical information, including medical history and health insurance information
  • Manages all internal and external communications concerning resident health services
  • Completes weekly caseload log and review with onsite providers, area leaders, and community clinical teams.
  • Assists in schedule management and coordination of services
  • Collaborates with the Nurse Practitioner on any areas of concern or successes voiced by the executive director and/or director of nursing through regular communication.
  • Communicates with and maintain a working relationship with physicians, care staff, the administrative team, and resident family members
  • Recording patients' vital signs, documenting physicians' notes and changes in medical conditions
  • Monitoring and documenting patient symptoms for physicians' review during clinical exams
  • Prepares immunizations and assists in various procedures under the direction of supervising provider
  • Comply with HIPAA and PHI guidelines and protect confidentiality at all times
  • Other clinical and administrative duties as assigned
Qualifications:
Qualifications, Education and/or Experience:
  • Graduate of an accredited medical assistant program with certification or registration a preferred
  • 2 years of relevant physician office experience with 1year of senior living experience a plus
  • Applicable state licensure(s) or certification
  • Exceptional communication and interpersonal skills are required along with the heart and passion to work with older adults while providing the best care possible
  • Ability to work well with others and take direction from management, as well as take initiative- willing to go above and beyond to ensure the needs of the organization are met in accordance with the company’s core values
  • Must have the ability to remain calm in stressful situations, be flexible, work well with many interruptions, and have exceptional multi-tasking skills
  • Respect for the principles of patient/resident rights and confidentiality
  • Must exhibit and promote a high level of professionalism, customer service, curiosity, and friendliness in all interactions with employees, residents, and visitors throughout the organization
Computer Skills:
Proficiency in Internet browsers (e.g. Explorer, Chrome, FireFox), as well as advanced knowledge of Microsoft Office programs: Outlook, Excel, Word and Publisher applications. Experience in working with various EMR and medical billing systems.
