POSITION SUMMARY: Provides a variety of support services to individuals and families who are homeless, near-homeless and/or in crisis; assists in coordinating services and communication among landlords and CNCAP; provides financial assistance for eligible persons; identifies and accesses client resources for persons; seeks to expand program and resources independently and confidently; promotes and maintains public relations; and completes forms accurately and timely. Responsible for maintaining, recruiting and enrolling persons in the HOPE (Hope Offers People Empowerment) case management program, Veteran’s RRH program, Supportive Services for Veterans & Families (SSVF) and Nebraska Homeless Assistance Program (NHAP)and Traditional Housing for Rural Independence, Viability, and Economic Stability (THRIVES).
ROLE AND RESPONSIBILITIES
- Responsible for completing agency intake form and inputting it into Clarity
- Provide ongoing support to clients and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed through the internal process to determine possible assistance through resources
- Create family personal plans to develop short and long-term attainable goals with clients
- Ensure files are complete with the necessary paperwork for client families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
- High school diploma or GED required (Bachelor's degree in human services or related field preferred)
- Valid driver's license
- 1-3 years of experience in human services or related field

SCHEDULE: Monday-Thursday: 7am - 5pm or Monday-Friday: 8am - 4pm
REPORTS TO: Housing Programs Coordinator
JOB STATUS: Full-Time, 40 hours per week
PAY: $16.50-$18.25 (depending on qualifications)
