Summary
The Case Manager is responsible for providing comprehensive case management services to clients by coordinating care, advocating for resources, and supporting individuals in achieving health, stability, and self-sufficiency. The ideal candidate is compassionate, organized, and committed to empowering clients through effective service planning and follow-up.
Key Responsibilities
- Conduct comprehensive assessments to determine clients’ needs, strengths, and barriers to care.
- Utilize motivational interviewing techniques to empower patients in their health journey.
- Develop individualized care plans in collaboration with clients, healthcare providers, and community partners.
- Connect clients to appropriate services such as medical care, housing assistance, food resources, mental health care, transportation, and employment support.
- Monitor and evaluate client progress; update care plans and provide follow-up services as needed.
- Advocate for clients to ensure access to necessary services and benefits.
- Maintain accurate and timely documentation in accordance with organizational and regulatory standards.
- Participate in team meetings, case reviews, and staff development activities.
- Establish and maintain effective relationships with patients, community agencies and service providers.
- Help with prior authorizations, specialty referrals, Patient Assisted Drug Program and hospital follow-ups.
- Become a Certified Application Counselor to assist with insurance applications on Georgia Access.
Requirements
- Bachelor's degree in Social Work or related field.
- Minimum of two years of experience in case management, social work, care coordination or a related role.
- Excellent communication, problem-solving and organizational skills.
- Ability to build rapport with diverse populations.
- Proficient in motivational interviewing techniques.
- Strong knowledge of community resources and social service systems.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in electronic health records and Microsoft 365.
- Successfully pass a background check and drug screen.
