Rivers of Hope is a compassionate, community-rooted home health agency providing services across Massachusetts. We specialize in Adult Family Care (AFC), Group Adult Foster Care (GAFC), Private Duty Nursing, Veteran Care, and Non-Medical Home Care. Our mission is to empower individuals to live with dignity and independence through personalized, high-quality care.
We are seeking a full-time Case Manager to support our AFC clients and assist with other programs as needed. This position also plays a key role in community outreach and marketing efforts to promote our services and strengthen partnerships.
Key Responsibilities
- Manage a caseload of clients enrolled in AFC and other programs.
- Conduct in-home visits for assessments, quarterly check-ins, and ongoing monitoring.
- Develop and maintain person-centered care plans in compliance with MassHealth standards.
- Document all activity accurately and promptly in WellSky (Kinnser) or other EMR platforms.
- Collaborate closely with RNs, caregivers, and the interdisciplinary team.
- Educate caregivers and clients on care plans, expectations, and available resources.
- Participate in internal audits, QAPI reviews, and care team meetings.
- Attend outreach events, community fairs, and referral meetings to represent the agency.
- Support marketing efforts by sharing information about services, distributing materials, and helping identify referral opportunities.
Qualifications
- Bachelor’s degree in Psychology, Social Work, Human Services, or a related field (required).
- At least 2 years of experience working with individuals with disabilities, elders, and/or veterans.
- Knowledge of AFC, GAFC, or community-based care strongly preferred.
- Proficiency with WellSky (Kinnser) or equivalent EMR.
- Strong organizational and interpersonal communication skills.
- Reliable transportation for fieldwork (mileage reimbursed).
- Bilingual in English and Cape Verdean Creole, Haitian Creole, Portuguese, or Spanish is a plus.
