Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. This role is vital in ensuring the delivery of trauma-informed, culturally competent, and individualized care plans that support youth in transitional and residential housing programs.
The Case Manager will play a crucial role in supporting youth and families by assessing their needs, developing personalized care plans, and facilitating access to necessary resources. This position requires strong interpersonal skills and a commitment to improving the lives of those we serve.
Duties
- Conduct comprehensive assessments to identify clients' needs and challenges.
- Develop, implement, and monitor individualized service plans in collaboration with clients.
- Provide crisis management and intervention to clients facing immediate challenges.
- Facilitate access to community resources, including healthcare, housing, and employment services.
- Supervise and mentor support staff as needed to ensure effective service delivery.
- Engage in motivational interviewing techniques to empower clients towards positive change.
- Maintain accurate documentation of client progress and case notes in compliance with organizational policies.
- Collaborate with multidisciplinary teams to coordinate care and support for clients.
- Advocate for clients within the child welfare system and other relevant agencies.
- Conduct assessments and create individualized service plans (ISTPs).
- Provide case management, crisis intervention, and advocacy.
- Facilitate Child and Family Team Meetings (CFTMs).
- Connect youth and families to community resources (healthcare, housing, education).
- Document progress and collaborate with DCYF, schools, and other partners.

- Strong supervisory skills — ability to lead, guide, and motivate support staff effectively.
- Crisis management expertise — skilled in de-escalation and rapid response to urgent youth and family needs.
- Motivational interviewing — ability to engage youth through strengths-based and client-centered approaches.
- Child welfare knowledge — understanding of child protection laws, policies, and system navigation.
- Behavioral health familiarity — knowledge of trauma, mental health, and therapeutic support systems.
- Experience in childcare or youth services — including group care, daycare, mentoring, or residential programs.
- Social work foundation — background or education in social work, human services, or a related field.
- Workforce development awareness — understanding of programs that promote employment, independence, and skill-building.
- Cultural competence — experience mentoring and supporting individuals from diverse racial, cultural, and socioeconomic backgrounds.
- Documentation and compliance — strong skills in case documentation, confidentiality, and working within state and program guidelines.
