Overview
Stable Family & Children Services LLC is committed to empowering families through supportive, community-based services that foster stability, healing, and growth. We are seeking compassionate and self-driven Subcontractor Case Managers to provide flexible, high-impact support to individuals and families across Northwest Indiana.
This position allows for flexible scheduling while fulfilling critical service needs in areas such as parenting support, crisis management, behavioral health, and community resource navigation.
Key Responsibilities
- Conduct comprehensive assessments to determine client strengths, needs, and service goals.
- Develop and implement individualized service plans in collaboration with clients and families.
- Use motivational interviewing and trauma-informed approaches to foster engagement and behavioral change.
- Provide timely crisis intervention and de-escalation support as needed.
- Coordinate services and make referrals to appropriate community resources, including housing, mental health, addiction services, and child welfare support.
- Monitor and document client progress, updating service plans to ensure client success and engagement.
- Facilitate and/or refer clients to parenting education, employment readiness, life skills coaching, and therapeutic services.
- Advocate for clients within systems such as child welfare (DCS), courts, healthcare, education, and employment.
- Ensure all documentation is completed accurately and in compliance with agency policies, state guidelines, and contractual expectations.
- Maintain timely records including case notes, service logs, releases, and incident reports.
- Follow agency procedures for privacy, confidentiality, and service quality assurance.
- Participate in supervision, training, and team development opportunities as scheduled.
Qualifications
- High school diploma or GED is required.
- Lived experience navigating systems such as child welfare, reentry, behavioral health, addiction recovery, or housing support is highly valued.
- Additional training, certifications, or coursework in human services, social work, psychology, or related fields is preferred.
- At least 1–2 years of experience in case management, peer support, crisis response, or related community services.
- Strong interpersonal and communication skills; able to build rapport with diverse individuals.
- Knowledge of local resources and familiarity with community-based support systems.
- Ability to work independently, manage time effectively, and maintain professionalism.
- Basic proficiency in using case management tools and technology.
- Must maintain a drug-free lifestyle and comply with a drug-free workplace policy.
- Subject to fingerprinting, CPI/CPS screenings, and local/state background checks as required by Indiana DCS and agency policy.
- Must possess reliable transportation for community-based and in-home service delivery.
Work Schedule & Pay
- Flexible Hours: Subcontractors may create their own schedules aligned with client and program needs (evening and weekend availability preferred).
- Pay: Competitive per-case or hourly compensation based on experience and assigned services. Rates discussed upon onboarding.
Job Types: Full-time, Part-time, Contract
Pay: $20.00 - $55.00 per hour
Expected hours: No more than 40 per week
