Case Manager – Your Home Project - Atlantic Homeless Alliance
The 'Your Home Project' Case Manager uses financial literacy education to provide counseling post home purchase. The role collaborates with AHA services and local non-profits to provide client-centered solutions to avoid homelessness.
What You’ll Do:
- Engage clients through housing counseling problem-solving techniques to identify available supports and prevent homelessness.
- Support clients in developing housing stability plans and budgeting to maximize their resources post purchase.
- Provide case management services, including individual, group, and family interventions, outreach, and support.
- Facilitate linkages to community social services, benefits, and other resources based on client needs and goals.
- Provide transportation support to clients per program guidelines and with supervisory consultation.
- Deliver skills development training to enhance clients' ability to maintain stable housing
- Provide in-home engagements to asses living arrangements and need for additional services and/or resources.
What We’re Looking For:
- Bachelor’s degree in Social Work, Psychology, Behavioral Health, Counseling, or Psychiatric Rehabilitation.
- At least 1 year of experience in social work, mental health, or a related field
- Strong communication, organization, and documentation skills
- Creative problem-solving mindset and a passion for housing justice
