Job Summary:
TASC of Northwest Ohio is seeking a dedicated and experienced professional to serve in a dual-capacity role as both a Case Manager and Diagnostic Assessor for justice-involved adults with behavioral health needs. This position is essential in supporting individuals involved in the criminal justice system by conducting comprehensive diagnostic assessments and providing strength-based case management services rooted in the national TASC model.
Key Responsibilities:
Diagnostic Assessment Duties:
- Conduct dual mental health and substance use assessments using the DSM-5-TR.
- Determine behavioral health diagnoses and assess level of care.
- Develop individualized treatment plans and coordinate referrals to appropriate providers.
Case Management Duties:
- Provide ongoing case management using strength-based, trauma-informed approaches.
- Coordinate care with probation officers, community providers, and treatment programs.
- Address barriers to care and support access to housing, employment, healthcare, and social services.
- Monitor client progress and adapt service plans accordingly.
Other Responsibilities:
- Maintain accurate and confidential documentation using electronic medical records.
- Apply principles of cultural humility, client self-determination, and trauma-informed care.
- Communicate effectively in both written and verbal formats.
- Work both independently and collaboratively as part of a multidisciplinary team.
Qualifications:
- Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
- Minimum of two years of experience in case management and diagnostic assessment.
- Proficiency with the DSM-5-TR and Microsoft Office applications.
- Familiarity with electronic health record (EHR) systems.
- Understanding of behavioral health systems, community resources, and criminal justice dynamics.
- Strong professional demeanor and commitment to confidentiality.
Compensation:
- Pay: $24.00+ per hour, commensurate with experience
