Overview
We are seeking a dedicated and compassionate Registered Nurse (RN)/ Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong commitment to patient care, with experience in various medical settings. This role requires proficiency in patient assessment, medication administration, and the ability to work collaboratively within a multidisciplinary team. The LVN will play a crucial role in providing high-quality care to patients across diverse populations, including those with developmental disabilities and geriatric patients.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct thorough patient assessments and monitor vital signs.
- Assist with procedures such as IV infusion, catheterization, and wound care.
- Document patient information accurately in electronic health record (EHR) systems.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Educate patients and their families on health management, medication administration, and post-discharge instructions.
- Maintain compliance with HIPAA regulations and infection control protocols.
- Participate in case management and discharge planning processes.
- Provide support in specialty areas such as hospice & palliative medicine, critical care, and pediatrics as needed.
Requirements
- Valid LVN license in the state of practice.
- Experience in medical office settings or long-term care facilities preferred.
- Proficiency with EMR systems such as Epic or Cerner is highly desirable.
- Familiarity with ICD-9/ICD-10 coding and medical terminology.
- Strong clinical skills including IV insertion, phlebotomy, medication administration, and basic life support.
- Knowledge of acute care practices, trauma medicine, geriatrics, and behavioral health is beneficial.
- Excellent communication skills and the ability to work effectively with patients from diverse backgrounds.
- Experience working with individuals with developmental disabilities or in memory care settings is a plus.
- Ability to pass background checks and drug screenings as required by facility policies.
