Position Overview:
We are looking for an exceptional Clinical Care Manager to lead and support our Long-Term Services and Supports (LTSS) Care Program in Holyoke, MA. In this dynamic leadership role, you will provide clinical consultation and guidance to Care Teams, ensure the delivery of high-quality care, and work on reducing member utilization of higher levels of care while improving health outcomes.

As part of our team, you'll also enjoy the flexibility of our hybrid remote work model—after six months of employment, you'll be eligible to work one day remotely per week and four days in the office. This schedule supports a healthy work-life balance while maintaining strong team collaboration.
Key Responsibilities:
- Lead and support Care Teams to ensure the best care outcomes for members.
- Guide and consult with Supervisors, Care Coordinators, and other team members on assessments, care planning, and interventions to improve overall health.
- Approve comprehensive health assessments and person-centered Care Plans for members.
- Collaborate with both internal and external service providers to meet members’ needs in an efficient, timely manner.
- Provide crisis management support and help reduce the risk of harm for members.
- Assist in transitioning members from higher levels of care to community settings, supporting their re-integration with the help of Care Teams.
- Review Critical Incident reports and ensure they are submitted promptly to stakeholders.
- Maintain thorough documentation within required timeframes and contribute to continuous quality improvement initiatives.
- Demonstrate cultural competence and health equity when working with a diverse and varied population.
- Respond to communications (emails, phone messages) within one business day.
- Participate in special projects as assigned by the Program Director.
Qualifications and Experience:
- Required License: Must hold a valid license at an independent practice level in one of the following disciplines:LCSW, LICSW, LMHC, LMFT, LADC I, RN with behavioral health expertise, PhD, OT, or Licensed Rehabilitation Counselor.
- Minimum of two (2) years of relevant professional experience in a clinical or behavioral health setting.
- Strong knowledge of mental illness, the interface between mental health and medical conditions, substance abuse, and care management.
- Proficiency with internet, word processing, and email systems.
Skills and Competencies:
- Strong leadership and supervisory skills, with the ability to effectively manage and guide a diverse care team.
- Ability to work collaboratively with internal and external partners to deliver integrated, person-centered care.
- Exceptional communication skills, both oral and written, with the ability to respond effectively to complex situations.
- Dedication to continuous learning and improvement of care management practices.
- Proficiency in using computer systems, including electronic health records and office software.
