Summary
We are seeking a skilled Case Manager to join our team in providing exceptional home healthcare services in the US. This role is pivotal in enhancing patient care and ensuring seamless access to necessary health services, aligning with our mission to improve the healthcare experience for our patients.
Responsibilities
- Coordinate care for the patient.
- Coordinate services and schedules patient visits.
- Follow Medicare and Medicaid regulations.
- Provide quality of care to patients in their homes according to policy and procedures.
- Demonstrate competence in documenting with Oasis.
- Observe, assess, and document patient symptoms and progress.
- Perform the initial home care visit and re-evaluate patient needs and progress when needed.
- Notify the physician of changes in the patient’s condition.
- Coordinate and monitor all patient care and services.
- Understand and perform the admission process plan of care.
- Educate patients and caregivers on disease process, medications, plan of care, and treatment plans according to the policy and procedure manual.
- Complete paperwork timely and efficient.
- Maintain patient records according to policy and procedure.
- Supervise LPN and HHA field staff as indicated.
- Participate in the on-call schedule.
Participate in and support quality improvement programs.
- Qualifications
- Registered Nurse
- A minimum of 1-2 years experience in healthcare with a working knowledge of commercial insurance, Medicare, and Medicaid reimbursement; DME experience is a plus.
- Working knowledge of automated billing systems, experience with Waystar, Wellsky, PracticeMate, and CMS preferred.
- Working knowledge and application of metric measurements, basic accounting practices, medical terminology, ICD-9/ICD 10, CPT, and HCPCS coding.
- Solid Microsoft Office skills required: Word, Excel, and Outlook
- Ability to independently obtain and interpret information
- Strong verbal and written communication skills
- Self-motivated with a proven ability to code with speed and accuracy
- Ability to multi-task
