Office Hours: Monday-Friday 8:30 am-5:00 pm

Position Summary

Clinical Case Manager – 50% of time

Under the general direction of the Program Director, the Part-Time Clinical Case Manager is responsible for providing individual case management and rehabilitative individual and group therapies as part of an interdisciplinary team. Bonita House Inc.’s Supported Independent Living (SIL) Service Team provides intensive outpatient mental health and case management services to individuals with co-occurring serious mental illness and substance use disorders who live in Alameda County.

Primary Responsibilities

  • Provide intensive outpatient case management, rehabilitative therapy and support to adults with mental health and/or substance use symptoms.
  • Help organize and lead social and recreational activities to help clients structure their time, increase social experience, and provide opportunities to practice social skills.
  • Must complete individual therapy, biopsychosocial assessments including ANSAs and sound clinical documentation that supports client treatment plans.
  • Actively participate as a team member, and present updates at each weekly team
  • Develop a keen understanding of Welligent and billing codes in order to document and bill MediCal appropriately and effectively assist in training new staff and
  • Assist staff with completing standard documentation requirements as needed and
  • Oversee therapeutic milieu including greeting members, ensuring they sign in/out, engages with members, coordinates daily room use schedule, and maintaining a safe and welcoming
  • Participates in clinical supervision with the agency QI Clinical Supervisor in order to ensure appropriate treatment and sign off on assessments as well as gain hours towards licensure if
  • Assisting at-risk Medi-Cal recipients or potential eligibles to understand the need for MH
  • Actively encouraging reluctant and difficult Medi-Cal recipients/potential eligibles to accept needed MH and healthcare services.
  • Operate a motor vehicle to include safe transport of clients to various appointments across Alameda
  • Non-SPMP gathering information about an individual’s health and MH
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures, meeting both Bonita House Inc and County documentation
  • Help clients (and family members) identify, understand, and combat stigma and discrimination associated with mental illness and develop strategies to reduce self-stigma.
  • Collaborate with the team to ensure the protection of clients’ rights in order to help clients to improve their knowledge of client rights and grievance or complaint procedures; support clients with filing, mediating, and resolving
  • Provide ongoing assessment, problem-solving, side-by-side services, skill teaching, support (prompts, assignments, encouragement), and environmental adaptations to assist clients with challenges in the areas of employment, housing, and activities of daily living.
  • Completed timely clinical documentation.
  • Participates in staff meetings.
  • Perform other duties as needed.

Qualifications and Requirements

  • Master's degree in psychology, Social Work, Counseling, or a related field & active registration with BBS required.
  • BHI conducts background checks on all candidates who have received a conditional job Satisfactory completion of background checks is a requirement for employment with the agency.
  • A valid driver’s license, reliable personal vehicle, personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are essential job requirements.

Employment Specialist – 50% of time

Under the general direction of the Program Director, the Employment Specialist is responsible to actively outreach, engage, and provide a full range of services to 10-15 adults in Alameda County who have mental health and substance abuse challenges as part of an interdisciplinary team. Bonita House Inc.’s Supported Independent Living (SIL) Service Team provides intensive outpatient mental health and case management services to individuals with co-occurring serious mental illness and substance use disorders who live in Alameda County.

Qualifications:

Required:

  • High School Diploma or Associated degree with 4 years of employment services
  • Demonstrated ability to work as an effective team
  • Knowledge of various job development strategies, local labor market trends in a range of occupational fields, and community resources.
  • Excellent computer skills including familiarity with Microsoft Office and the ability to learn specialized databases and software including Electronic Health Record (E.H.R.) systems.
  • Understand and follow HIPAA and PHI guidelines and
  • Ability to maintain a high level of confidentiality, a professional demeanor and to represent the organization in a positive manner at all times.
  • Oral and written communication—speaks clearly and persuasively in positive or negative situations, demonstrates group presentation skills and conducts Completes written progress notes and other formal communications skillfully and professionally.
  • Demonstrated ability to be culturally sensitive and respect
  • Must have a valid California Driver License, a driving record acceptable by the Agency’s insurance company; own a vehicle that you are willing to use as part of your employment and proof of personal vehicle
  • Must be able to meet the Immigration Reform Act of 1986

Preferred:

  • Lived experience
  • Mental Health Rehabilitation Specialist (MHRS);
  • Prior sales, marketing, or coaching experience
  • Bilingual in English/Spanish, or other languages
  • Knowledge of regional workforce development and social service systems is strongly

Essential Functions/Skills:

  • Engages clients and establishes trusting, collaborative relationships directed toward the goal of competitive employment in community job settings.
  • Develops an individual employment plan with a career profile with the client and other treatment team providers and updates it quarterly.

  • Conducts job development and job search activities directed toward positions that are individualized to the interests and uniqueness of the people on the caseload.
  • Assesses clients' vocational functioning on an ongoing basis utilizing background information and work experiences.
  • Assists clients in obtaining information about their benefits (e.g., SSI, Medicare, ) and how they will be affected by employment in order for clients to make good decisions about employment opportunities.
  • Comply with all state Department of Rehabilitation (DOR) regulations and CARF standards, as well as all agency policies and procedures.
  • Assist clients explore educational and vocational training opportunities with a focus on job
  • Conducts job site analyses and makes independent and calculated decisions regarding an employer’s appropriateness for Maintains a highly effective knowledge base regarding the Americans with Disabilities Act (ADA); Educates employers on the ADA and the benefits of employing quality employees with disabilities; identify any potential barriers and work to provide solutions.
  • Provides education and support to employers as agreed upon by clients, which may include negotiating job accommodation and follow-along contact with the employer.
  • Provides outreach services as necessary to clients when they appear to disengage from the Uses a variety of methods to provide outreach.

Physical Requirements

Physical demands include continual sitting, typing, and repetitive computer use for up to 8 hours a day. The position also requires frequent communication with others, including hearing and speaking clearly with individuals and groups, in person and via phone and video calls. Ability to lift 15 pounds at times. Regular local travel and operation of a motor vehicle is required. These requirements are representative of those that must be met to successfully perform the job’s essential functions.
