Job Title: Community-Based Case Manager

Please note: The home where this position is located is approximately 1 hr. from St. Louis. The residential home is in northwest Jefferson County. The largest city near it is House Springs. The exact location is undisclosed due to the confidentiality of the clients.
Primary Objectives: To ensure the external needs of youth in our care are met, and to serve as an advocate for our youth in multi-disciplinary settings.
Primary Responsibilities: The Case Manager’s primary responsibility is to coordinate medical, legal, and community services for youth in our care. The Case Manager will provide services for residential and community-based youth, including but not limited to:
- Meeting with the youth to develop a treatment plan with SMART goals.
- Identifying community resources that would be a benefit to the youth.
- Scheduling, facilitating, and following up on regular treatment team meetings.
- Advocating for the youth in external meetings (including legal, educational, and placement meetings).
- Accompanying the youth to external medical appointments.
- Serving as the primary point of contact for the youth with outside providers.
- Communicating with staff regarding youth progress and youth needs.
- Compiling, maintaining, and reporting on program outcomes. Complying with all local, state, and federal policies, rules, and regulations.
Additional Responsibilities:
- Representing The Covering House in a positive and professional manner.
- Maintaining the highest standard of confidentiality.
- Exhibiting knowledge of abuse risk management and peer to peer abuse prevention.
- Functioning as a team member in all interactions with residents and staff.
- Serving on internal committees as assigned.
- Actively participating in staff development activities.
- Providing education on sex-trafficking as needed to enhance youth care.
- Other job assignments as instructed by supervisor(s).
Qualifications:
- Bachelor’s degree in social work, psychology or related field is required or 4 years of direct related experience is required.
- A minimum of 21 years of age, with a demonstrated capacity to listen to adolescents with understanding, help them find constructive solutions to problems, and collaborate with peers for service delivery.
- JIREH certified (crisis intervention and physical intervention).
- Moderate skills with Google and Microsoft Office Suite.
- May not have a substantiated report of child abuse or neglect.
- Willing to obtain a valid Class E driver's license. This employee must be able to transport clients in accordance with licensing standards.
- Must adhere to a comprehensive background check.
- Must fill out a medical release form and confidentiality form.
