Pay Range: $17.46 - $22.07
This is a paraprofessional high school graduate level position designed to assist persons who meet medical necessity criteria for community living supports, employment, and/or skill building services. The position incumbent will provide a wide range of direct assistance to consumers (within the scope of his/her training) and within the framework of the consumers Individual Plan of Service (IPOS). The services provided are designed to increase or maintain personal self-sufficiency and support recovery.
JOB DUTIES:
- Skills Building.
Education and training including instructions about treatment regimens and use of assertive technology and/or medical equipment that are needed to safely maintain the person at the home specified in the IPOS. Is a coach and a model to reinforce adaptive, pro-social behaviors. Assist in building coping skills to manage symptoms of mental illness, substance use, and other needs as discussed in treatment goals.
- Monitoring.
Defined as reviewing the response of treatment as defined in the IPOS and the status of the person’s needs and how this affects their day-to-day life and reporting this to treatment team as necessary
- Support Services.
Support provided one-on-one or in a group to aid in identifying coping strategies for completion of activities of daily living, social interaction, community setting supports, and other needs as indicated in the person-centered plan. This may also be done in conjunction with the consumer’s natural supports or other treatment providers.
- Transportation.
Will transport consumers to and from a variety of appointments both internal to the agency and within the community as medically necessary
