SNF DIALYSIS RN – JOB DESCRIPTION
Reports to: _Clinical Ops Manager (DON) and Field Manager (ADON) Exemption Status: Non-exempt
PRN POSITION- may be scheduled per communicated availability at any of the facilities in the proximity of the San Fernando Valley (Reseda area, North Hollywood, Montclair, etc...)
OVERVIEW OF THE JOB
SNF Dialysis Floor RN is responsible to assist in the supervision of patient care and performing certain duties requiring an RN license during the dialysis treatments, under the direction of the Site HD Charge Nurse Manager.
MINIMUM QUALIFICATIONS
(Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required):
- Licensed nurse in the state of assigned locations
- Minimum one (1) year of nursing experience; minimum of six (6) months of hemodialysis experience preferred
- Fluent in the written and verbal English skills necessary to successfully perform all duties and responsibilities of this position.
- Ability to be available to work during all hours of patient training and/or treatment
- Possess valid driver’s license. Able to drive a vehicle and have reliable means to travel to assigned location. Driving record must meet insurance liability requirements.
- Self-motivated and able to function independently
- Passion for quality care, education and the individual team member’s appropriate role in the provision of care
- Ability to prioritize and adapt based on changing priorities
- Ability to proactively identify and analyze clinical and financial outcomes
- In-depth understanding of facility operations and the roles of direct caregivers
- Solid reasoning, critical thinking, problem solving abilities
- Able to delegate appropriately
- CPR certification required
ESSENTIAL JOB FUNCTIONS
1. Accountable for the provision of outstanding customer service to all external and internal customers (skilled nursing(SNF) administration patients, staff, physicians)
2. Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
3. Equipment and inventory management oversight to assure the safety of patients and staff, as directed by the Site HD Charge Nurse Manager
4. Ensuring that the company meets all ESRD regulatory requirements and the practice of continuous Quality Assessment Process Improvement (QAPI) , as directed by the Site HD Charge Nurse Manager
5. Responsible for assisting Site HD Charge Nurse Manager in ensuring that all employees receive appropriate training according to policy, including training to ensure ongoing compliance with all risk management initiatives and quality performance measures.
6. Responsible for addressing and acting on adverse events and action thresholds
7. Ensure all policies and procedures are communicated to and implemented by dialysis personnel
8. Assistance in ensuring the integrity of the medical records and other administrative / operation records
9. Responsible for compliance with all data collection and auditing activities, as directed by the Site HD Charge Nurse Manager
10. Oversee and ensure the integrity and safety of the facility water systems where applicable
11. Address patient concerns and questions regarding dialysis care provided and to resolve patient issues
12. As directed by HD Charge Nurse Manager, assistance in completion of patient care plans through organized meetings of the healthcare team to discuss patient plan of care
13. Assisting in daily oversight of patient census and staff schedule to maximize financial efficiencies
14. Responsible for assisting in coaching, developing and overseeing performance of dialysis personnel
15. Assists HD Charge Nurse Manager in directing, orienting and training new RN and other personnel in leadership, management of the facility staff, policy and procedure and QAPI responsibilities. Provides proactive feedback to HD Charge Nurse Manager regarding all staff members, offering support, encouragement and professional growth as appropriate
16. Ensures compliance with Human Resources policies and procedures
17. Responsible for implementing and driving teamwork within the assigned area and within assigned facility
18. Ensure a strong communication process between dialysis personnel and all support personnel at assigned locations.
19. Responsible for strong Medical Director and physician relationships
20. Assist management when requested to review and approve timesheets, exception reports and mileage expense forms of all assigned personnel
21. Ensure patient and treatment charge information and other related data are accurate and submitted to the corporate office in a timely manner
22. The ability to be physically present onsite.

4. Attends all required in-services and training programs
5. Understands and follows patient and employee policy and procedure

2. Physical requirements can vary. However, in general, the position requires the following physical activities:
o Ability to lift minimum of 1 pound to a maximum of 35 pounds unassisted, and able to perform repetitive standing, sitting, stooping, walking, stretching, reaching, be able to stand on feet for extended periods, and use full range of body motions

Pay: $48.00 - $52.00 per hour
Expected hours: No more than 39 per week
