Key Responsibilities:
- Support individuals in developing and maintaining daily living skills to enhance independence
- Coordinate, schedule, and attend medical and other essential appointments with individuals
- Assist in accessing and navigating community resources and services
- Promote mental and physical wellbeing through positive routines and engagement
- Encourage participation in cultural and faith-based activities that align with individual preferences
- Foster the development of healthy coping strategies and resilience
- Provide support with problem-solving and decision-making in day-to-day life
- Maintain clear, accurate documentation and communication with relevant team members and stakeholders
Qualifications & Skills:
- Experience in disability services, community support, or a related field (preferred)
- Experience with Positive Behavioral Supports
- Strong interpersonal skills with a respectful, person-centered approach
- Ability to work independently and as part of a team
- Familiarity with cultural competency and trauma-informed support practices
- Reliable transportation and flexibility to travel locally with clients
- First Aid, CPR, and relevant clearances (or willingness to obtain)

Primary Responsibilities:
- Assists individuals in health support, safety, grooming, household care, meal preparation, ambulation, and supervision.
- Assists with attaining independence goals as described in the individual’s ISP to progress with their independence as adults.
- Produces accurate documentation of the services delivered. Documents care provided and response of the consumer/family. Submits time sheets and all other documentation in a timely manner.
- Performs all duties in an ethical manner and in compliance with local, state and federal laws, Medicare/Medicaid regulations and established personnel policies and procedures, including confidentiality and HIPAA guidelines.
Physical Demands:
- This position requires the employee to work in a variety of environments (housing styles, cleaning and maintenance, heating, and cooling) during visits to participants in their place of residence.
- Driver’s License Preferred
Knowledge and Experience:
- High school diploma or equivalent.
- Minimum age requirement of 18 years.
